DISTINGUISHING CHARACTERISTICSCommunity Health Workers work with moderate to difficult client cases and exerciseconsiderable independent judgment in the handling of their day-to-day interactionswith clients. CHW may also be involved in street outreach and may be in contactwith individuals and families in crisis who may be unhoused, experiencing healthissues, using alcohol and drugs, and who may not be attentive to basic personalhygiene, health and safety practices. CHW may also be required to work flexiblehours.SUPERVISION RECEIVED AND EXERCISEDGeneral direction is provided by a higher-level management position within thedepartment.Typical DutiesMeet with clients in their homes, physician offices, Serenity Health Services,community members offices, street outreach, or other agreed upon public placesto introduce programs and coordinate services.Collaborates with Lead Care Manager to develop individualized treatment plan.Promote awareness of social determinant health issues and the commitment torecovery, wellness, and self-help.Conduct community outreach to raise awareness of available health, behavioralhealth, and social services programs.Develops trusting relationship with patients by providing support and advocacy tohelp achieve health goals.Provide client field-based services, education, and street outreach.Assists a variety of clients in accessing health, mental, social-related services, andcommunity resources; such as accompaniment to specialist appointments andassistance with enrollment forms as needed to also include transportation.Facilitates communication between all parties (patients, families, colleagues, andcommunity-based organizations) as needed.Attend meetings and trainings as required.Communicate, represent, and promote the peer and family/caregiver perspectivewithin the healthcare system.Offer encouragement, mentoring, and advocacy; and perform problem-solving on aone-to-one basis to help clients obtain the necessities of daily living.Educate client on approved methods of preventive, supportive, and rehabilitativehealth care.Acting as a liaison/advocate for clients in obtaining medical, mental health careand/or other supportive services.Assist consumers with navigation of the Merced County Integrated Service deliverysystem and/or other delivery systems, in order to ensure needs are met by theappropriate caregiver.Maintain and ensure HIPAA, ethical standards, and professional boundaries with allstaff and clients.Contribute to an organizational culture that respects and celebrates the diversity ofall clients.Will participate in Multidisciplinary Treatment (MDT) Teams.Document client services and interactions in accordance withguidelines, policies, and procedures.Perform a variety of clerical tasks including data entry to support daily operations.Essential job duties may be assigned that are not listed above but are relativeto this job classification. Programs and services that support and care for persons with complexhealth care needs. Concepts of the physical, emotional, and social needs of justice-involvedindividuals. Community resources and techniques for building rapport with clients andfamily members.Skill/Ability to: Work and communicate effectively with people with diversebackgrounds by respecting beliefs, interpersonal styles, attitudes, and behaviors of both clients and co-workers.Coordinate services for clients of vulnerable populations, including but notlimited to unhoused and justice involved. Operate contemporary office equipment inclusive of computer, keyboard,and all applicable electronic equipment. Relate and communicate effectively with peers, family members, and the community served. Relate to clients through sharing of success stories. Understand and follow verbal and written instructions. Retain and recall information pertaining to specific procedures. Lead and interact with clients/consumers in social, recreational, and living skills activities. Record information accurately in the Electronic Health Records system Complete multiple priority projects with strict deadlines. Maintain confidentiality of patient/client information..
